An urgent hunt is underway after an elderly grandmother was seriously injured in an unprovoked attack in the carpark of a fast food restaurant.

The woman, 84 remains in hospital undergoing surgery following the alleged assault in Midland in Perth’s east at about 12.30pm on Saturday.

Police allege she was confronted by another woman in the car park, who knocked the grandmother to the ground before fleeing the scene.

The grandmother was rushed to Royal Perth Hospital with head injuries, where she’s in a serious but stable condition.

Her prognosis remains unclear, according to her daughter.

‘We won’t know until after surgery. She needs to survive the operation, then there’s the recovery,’ she wrote online on Sunday night.

Detectives have a second day scouring CCTV trying to establish a motive for alleged assault. 

Western Australian Police have released an image of the woman they believe can assist with their inquiries.

She is described as 160cm tall, medium build with short brown hair and brown eyes. 

She was wearing a black T-shirt and shorts at the time.

Police urged anyone with information to contact Crime Stoppers.
