New details have emerged in the investigation into the death of a woman with a severe disability in Port Augusta which police have declared a major crime.

Twenty-six-year-old Tegan Fredericks was found dead in her home in South Australia, sparking an investigation into potential criminal neglect.

The conditions inside her home were described by police as “not fit for human habitation”.

“These circumstances are very distressing and concerning and it is for that reason that a thorough police investigation is underway,” Police Minister Dan Cregan said. 

“As I understand it, it’s likely there will be a coronial investigation.”

Questions have also been raised about why Fredericks was not being cared for with the assistance of the NDIS.

“In 2017, there was an initial expression of interest to be on the scheme then documentation was sent back and then nothing more was ever heard,” NDIS Minister Bill Shorten said.

Authorities are now examining who was aware of Fredericks’ circumstances and whether there are gaps in the disability sector.
