Five people were injured and treated at the scene, including a police officer, following the two-vehicle crash and alleged stabbing in Engadine.
Police have charged the man with eight offences, including causing grievous bodily harm with intent to murder, reckless wounding, dangerous driving and assault occasioning actual bodily harm.
The man was refused bail and is set to appear before Sutherland Local Court today.
Emergency services were called to the intersection of Princes Highway and Old Bush Road in Engadine about 9am on Sunday following reports of a crash and stabbing.
A white Jaguar and blue Mitsubishi Lancer had collided and the man, 58, was seen allegedly fleeing the crash scene.
He was taken into custody after officers deployed a laser.
Police confirmed a male constable suffered serious lacerations to the left wrist which allegedly occurred during the arrest.
A 48-year-old woman, who was a passenger in the Jaguar, was found with multiple stab wounds.
The man and woman were in a relationship, police said.
Police allege she was stabbed in the moments leading up to the crash, which is why he was driving so erratically.
Another man, 67, suffered stab wounds to his abdomen and the female driver of the Lancer, 20, sustained a broken wrist.
All five were treated at the scene before being taken to St George Hospital and Liverpool Hospital.
The 48-year-old woman and a 67-year-old man remain in St George Hospital, both in stable conditions, while the police officer and male passenger in the lancer were released from hospital.
The 58-year-old man, who is known to police, remains in a stable condition under police guard at Liverpool Hospital.
He was charged today with causing wounding/grievous bodily harm to person with intent to murder domestic violence related, wounding person with intent to resist/prevent arrest, reckless wounding and assault occasioning actual bodily harm.
He was also charged with dangerous driving occasioning grievous bodily harm, failing to stop and assist after vehicle impact causing grievous bodily harm, affray and resisting police in execution of duty.
Officers from Strike Force Valdez are continuing to investigate the incident.
